ALS Care Grant Program

Dear Friends: 

The ALS Association regularly reviews the services and programs offered to those living with ALS and their family members to ensure our programs have maximum benefit(s) to all those served by the ALS Association. Procedures to access each program are likewise reviewed and updated with the goal of improving equity, access, and convenience through reducing the paperwork load for caregivers and staff.

Recipients must be a resident in the state of Ohio.
Recipients must be registered with The ALS Association and reside in designated counties.
Recipients may not combine funds with other Ohio grants using funding through the American Rescue Plan Act.

Therefore, to ensure equitable access to financial assistance to all people living with ALS/MND and to be as inclusive as possible, the ALS Association has made the decision to create ONE GRANT APPLICATION FORM that will be used to request financial assistance.

The ALS Grant Application will allow requests for financial support for:  

  1. Hired care and respite services (provider must NOT be a resident of the home)  
  2. Home and vehicle modifications 
  3. Medical equipment, medications, and copay/co-insurance/out-of-pocket costs  
  4. Access to ALS specialty care/clinic, physician, allied health professionals and clinical trials 

Funding for the ALS Grant is limited to a maximum of $2000 per year per client. Grants are provided based on availability of funds. There is no guarantee, entitlement, and no promise/entitlement of funding. Grant funding is not provided in return for any services, reviews, or participation in awareness, public policy, or volunteer role(s).  

The ALS Association Grant assists with the care and needs of those living with ALS/MND. The ALS Association assumes no responsibility or liability for any direct or indirect services, products, or client care or for the care arrangement and/or business relationships between the person living with ALS/MND (including their loved ones) and their selected product or service provider. The ALS Association assumes no fiscal responsibility beyond the approved grant amount.
